Insurance
As a courtesy to our patients, we will file a claim for your
dental visit. If you are assigning benefits payable to the
Doctor, please be aware that you are responsible for any
deductible, co-payment, or balance not covered by your plan
at the time service is rendered. You will also be
responsible for all collection costs if your balance is not
paid within 45 days of service.
We suggest that you familiarize yourself with your
individual plan. All dental plans have deductibles,
co-payments and yearly maximums. It is virtually impossible
for this office to know this information for each and every
insurance plan offered by employers. We will be happy to
work with you and provide you with approximate information
of payments which may be expected. Most insurance companies
will not guarantee payment until the actual claim has been
submitted or a pre-estimate of services to be done is sent
prior to your dental treatment.

Finally, financial arrangements for extensive dental
treatments may be arranged with the Office Manager, prior to
the start of your proposed treatment. This option is not
available for routine cleanings, x-rays, examinations, and
filling restorations.

Checks and cash
are also accepted. The patient is liable for a 1.5% monthly
service fee on all past due accounts as well as all legal
and collection fees.
We also accept CareCredit for any or all of your dental procedures. CareCredit is a personal line of credit for healthcare treatments and procedures for your entire family. It works like a credit card but it has two advantages.
